Second Annual NYU Seminars in Gastroenterology
An Intensive course for specialists, internists, and allied health professionals
Saturday, April 1, 2006
NYU Medical Center
New York, New York 10016

The New York University (NYU) Seminars in Gastroenterology have been developed to examine some of the most recent trends, issues, and controversies in the field, for the advancement of practicing gastroenterologists.

Clinician understanding of the genetic basis for hereditary colorectal cancer syndromes is improving, but with this comes additional issues. A predisposition to other secondary malignancies is being recognized. Revised guidelines from the National Cancer Institute may provide a way to overcome the challenge of identifying patients at risk, and for further selection of patients for genetic testing. Data from studies such as the National Polyp Study may offer some guidance on patient screening, monitoring, and surveillance.

With the growing epidemics of obesity and diabetes, understanding the role of visceral fat and inflammation in disease states, including fatty liver disease, is key. It is anticipated that fatty liver disease will be a frequent problem and a challenge for hepatologists in the future. A better understanding of disease mechanisms may help to guide treatment and patient management. Also in the liver, there is an increased urgency to identify and treat chronic hepatitis C earlier, to minimize the complications resulting from disease, including hepatocellular cancer. Alternative treatment strategies are needed for nonresponders to pegylated interferon, ribavirin and other difficult-to-treat populations.

Also on the forefront of gastroenterology are new applications for existing technology, including endoscopy, with uses evolving in neuro-oncology, transgastric surgery, and anti-reflux surgery and anti-reflux procedures. With the mechanisms of inflammatory disease being further elucidated over the past several years, newer treatment strategies for inflammatory bowel disease are emerging, including biologic therapy, probiotics and dietary approaches, among others.

The curriculum is intended to provide participants with an overview of the effects on cancer, management techniques, and current treatment methodologies in order to give physicians the tools to provide optimal care. Our nationally recognized faculty will discuss topics such as pain management; diet; telemedicine; antiangiogenesis; lymphoma treatment; implications of the human genome on cancer care; elevated PSA levels; cancer screening in primary care practices; management, treatment, and genetic counseling for breast cancer; complementary, alternative, and multimodality therapies; and talking to patients about a cancer diagnosis. Meet the Professor sessions enhance the educational value of this course.

Angiotensin II and Vascular Disease
A Link Between Endothelial Dysfunction, Atherosclerosis, and Hypertension?

This symposium will reveal the details of one of the largest and most ambitious clinical trials designed to investigate the role of an angiotensin II receptor blocker (ARB), an angiotensin converting enzyme (ACE) inhibitor, and a combination of the two in the prevention of stroke, myocardial infarction, and cardiovascular death. Both ARBs and ACE inhibitors are known to reduce blood pressure through interruption of the renin-angiotensin system. The Heart Outcomes Prevention Evaluation (HOPE) study has recently shown that the ACE inhibitor ramipril reduced the risk of a primary cardiovascular event in high-risk patients by 22%. The ONTARGET (ONgoing Telmisartan Alone and in combination with Ramipril Global Endpoint Trial) study described in this symposium will evaluate the value of the ARB telmisartan for the prevention of cardiovascular morbidity and mortality in comparison to, and in combination with, ramipril. This 5-year outcome study will enroll approximately 30,000 patients in 35 countries. The esteemed symposium faculty will also discuss the proposed mechanisms by which 24-hour blood pressure patterns and agents that affect the renin-angiotensin system impact cardiovascular morbidity and mortality.

Emerging Strategies Using Combination Therapy to Achieve Target Blood Pressure: A Focus in High-Risk Populations
This symposium highlights the need for aggressive treatment of hypertension as a means to bring patients to the target blood pressure values set by the JNC VI and other consensus groups. Epidemiologic surveys such as the NHANES III show that only a minority of hypertensive patients have acceptable blood pressure control rates. Combination therapy is emerging as an attractive means to achieve improved blood pressure control in these difficult-to-treat patient groups. The purpose of this symposium is to review the variety of combination agents available for the treatment of hypertension and to discuss the treatment strategies needed to control blood pressure in high-risk hypertensive populations. The information conveyed in this symposium is intended to result in enhanced patient care that will help to improve the statistics on control of blood pressure.
